    Summary

    The Health Foundation of South Florida's (HFSF) mission is to invest in and be a catalyst for collaborations, policy and systems change that improve the health of South Florida communities, with a focus on vulnerable, low- to moderate-income populations. HFSF's vision is for all South Floridians to have an equal opportunity to achieve optimal health and well-being. 


    HFSF has the following focus areas:

    • Access To Care: By increasing access to routine and preventive medical care for children and adults, it is their goal to improve overall healthcare availability and utilization among uninsured and underserved communities.
    • Health & Social Needs: To reduce health disparities, we will advance a collaborative system, across sectors, that enables and facilitates the integration of social needs and health care.
    • Thriving Community: With an overall objective to improve the social and economic conditions that promote physical, mental and emotional well-being, they seek to foster a thriving South Florida by creating awareness about the intersection of community development and health, with a special focus on housing and economic prosperity issues.
    • Racial Equity: At the center of their work improving the health of South Florida communities is a commitment to advancing health equity. They are dedicated to addressing the structures and systems that contribute to health disparities among people of color by integrating racial equity and inclusion into all their functions and asking the same of our partners.
    • COVID-19: As the global health community and the South Florida region continue to come together to confront the COVID-19 pandemic, the Health Foundation of South Florida has responded. Every decision they've made and continue to make is guided by their long-standing commitment and mission to ensure everyone in their community has the opportunity and resources they need to live a healthy life.
    • Other Community Needs: We are open to a variety of ideas from the community that align with our mission to improve health and well-being in Broward, Miami-Dade, and Monroe Counties, especially among low-income and minority communities.

    • The project is supported by other private, public and/or community resources.
    • The project's community benefits continue beyond a grant from the Health Foundation.

  • Eligibility Details

    To be eligible for a grant from the Health Foundation of South Florida, applicants must be a tax-exempt nonprofit under section 501(c)(3) of the Internal Revenue Code, or a local or state governmental agency. In addition, the project for which funding is being requested must exclusively serve the residents of Broward, Miami-Dade and/or Monroe Counties.
